Once complete, print, sign by hand and bring printout to the Hawaii meeting on Tuesday, October 20th. Please also bring copies of the front and back of your insurance card or uninsured waiver. Also if your student has injectables please complete the form for that condition (print and fill out by hand). The PDF has links to all of the documents you may need.
